In the past, clearing such a blockage meant major surgery. Today, a remarkable procedure called ERCP allows doctors to remove bile duct stones and relieve blockages through the mouth, without any external cut at all. Understanding what ERCP is and when it is needed can be reassuring for patients facing this situation.<\/p>\n<h2>Bile Duct Stones Versus Gallbladder Stones<\/h2>\n<p>Many people are familiar with gallstones, which form in the gallbladder. Bile duct stones are related but different. The bile duct is the tube that carries bile from the liver and gallbladder into the intestine. Sometimes a stone forms in or slips into this duct and blocks the flow of bile. This is more serious than a stone sitting quietly in the gallbladder, because the backed-up bile can cause jaundice, infection and inflammation of the pancreas.<\/p>\n<h2>Recognising Cholangitis: A Medical Emergency<\/h2>\n<p>When a blocked bile duct becomes infected, the condition is called cholangitis, and it can become life-threatening if not treated promptly. Doctors look for a classic combination of features known as Charcot&#8217;s triad:<\/p>\n<ul>\n<li>Pain in the upper right part of the abdomen<\/li>\n<li>Fever, often with chills and shivering<\/li>\n<li>Jaundice, the yellowing of the eyes and skin<\/li>\n<\/ul>\n<p>Anyone with this combination needs urgent medical attention, because relieving the blockage quickly can be lifesaving.<\/p>\n<h2>How ERCP Works<\/h2>\n<p>ERCP stands for endoscopic retrograde cholangiopancreatography, a long name for an elegant procedure. A flexible endoscope is passed through the mouth, down the food pipe and stomach, to the point where the bile duct opens into the intestine. Using X-ray guidance and dye, the doctor can see the duct clearly. A small cut is then made at the opening of the duct, a step called sphincterotomy, allowing the stone to be extracted with tiny instruments. If needed, a small tube called a stent can be left in place to keep the duct open and draining.<\/p>\n<h2>When ERCP Is Preferred Over Surgery<\/h2>\n<p>ERCP is the treatment of choice for clearing stones from the bile duct because it avoids open surgery and its associated recovery. It is especially valuable in cholangitis, where the priority is to relieve the blockage and drain the infection rapidly. In many patients with both gallbladder and bile duct stones, the bile duct stones are first cleared by ERCP, and the gallbladder is removed by keyhole surgery afterward, giving a complete and well-staged treatment.<\/p>\n<h2>What to Expect During and After<\/h2>\n<p>ERCP is performed under sedation, so you are comfortable and relaxed throughout and usually remember little of it. The procedure typically takes thirty minutes to an hour, depending on what needs to be done. Afterward you are observed for a few hours, and many patients can go home the same day or after an overnight stay. A sore throat or mild bloating for a short time is normal, and most people recover quickly and return to normal activities soon after.<\/p>\n<h2>A Gentler Path to a Serious Problem<\/h2>\n<p>Blocked bile ducts and cholangitis are serious, but ERCP has transformed their treatment from major surgery into a precise, minimally invasive procedure.
